- What it is: A proactive document outlining your loved one's preferences and established care plans before a crisis occurs.
- When to use it: Fill this out during stable periods to document baseline behaviors, de-escalation strategies that work, and preferred treatment facilities.
- Why it helps: Gives you a clear roadmap to follow when stress levels are high, ensuring your loved one's voice is respected.
🚨 2. Crisis & Emergency Handout for First Responders
- What it is: A ultra-brief, one-page cheat sheet designed for police, EMTs, or mobile crisis teams.
- When to use it: Hand this physically to first responders the moment they arrive at your home during an emergency.
- Why it helps: First responders need fast, actionable facts. This immediately alerts them to diagnoses, specific triggers, calming techniques, and whether there is a history of violence or trauma, keeping everyone safer.
